• You must, without exception, complete all new patient forms and consents. 

  • It is the patients’ (or facility’s) responsibility to contact our office for all controlled substance prescription refills.

  • Please allow ten (10) business days to refill all controlled substances, and 48-hours to refill all maintenance mediations.

  • To refill maintenance meds, please contact your pharmacy - allow 48 hours for refills. If you need a 90-day prescription, please notify your pharmacy. 

  • If your insurance requires a prior authorization for any testing or medications, please allow ten (10) business days. 

  • If you leave a message for the ARNP or the office assistant, please allow 24 hours for a return call. 

  • Messages left after 2pm will not be returned until the following business day. 

  • Any paperwork that our office is requested to complete on your behalf may take up to seven (7) business days to process. 

  • To access patient information through our Medical Records, please contact the office. You must be on file as the patient or the patient’s responsible party. You will need to provide an email address.

  • Dogs MUST be restrained before Practitioner arrives for visit
